Membership Paperwork
All recruitment and new member information must be submitted via the online membership paperwork form. This form MUST be completed with all dates, times and locations, as well as provide officer and advisor information PRIOR to your meeting with the Fraternity and Sorority Life staff. All new member calendars/syllabi and recruitment calendars MUST be included in the form. The Verification of New Members form must still be submitted online no later than two business days after invitations to join are accepted.
Membership Paperwork
Please complete the following paperwork with accurate and detailed information. The information submitted will only be shared with the Fraternity and Sorority Life staff working with membership. In addition to completing the Membership Paperwork form, you are expected to schedule a 20-minute meeting with an FSL Staff Member PRIOR to any membership activities, as mentioned in Section 2 of the Procedures for Intake/Recruitment and New Member Activities.
- Step One: Complete the Membership Paperwork Form (Completed before Intake/Recruitment/Rush begins)
- Step Two: Add supplementary materials to the chapter Membership Paperwork folder on Google Drive (search by using 2-3 digit chapter code and “Membership Paperwork 2023-2024”
- New Member OR Intake Calendar (can use template or upload your own syllabus with schedule)
- Interest Meeting Marketing Materials, if applicable
- New Member Presentation Show Form
- New Member Bill of Rights
