Membership Paperwork


The Department of Fraternity and Sorority Life is on Google! No more forms to complete and calendars to turn in–at least not on paper. All recruitment and new member information must now be submitted via the online Membership Paperwork Form. This form MUST be completed with all dates, times and locations, as well as provide officer and advisor information PRIOR to your meeting with the Fraternity and Sorority Life staff. All New Member Calendars/Syllabi and Recruitment Calendars MUST be shared with the Department of Fraternity and Sorority Life via the Membership Google Drive. To do this, simply share your document(s) with the email address The Verification of Aspirants and Verification of New Members forms must still be submitted in hard copy no later than two (2) business days after invitations to join are accepted.
